Welcome to the office of Gregory J. Toone, DDS. Please download and fill out these forms before your first appointment. 

Dental Payment Information

We will file your dental insurance claims for you to make the process easier and to maximize your benefits. We also accept the following payment methods:

  • CareCredit
CareCredit is a third-party financing option that will help make paying for your dental care easier, offering no-interest and a long-term payment option. For further information, ask our front desk staff. You may also learn more about our office here. 

At Toone Family Dentistry, we’re dedicated to providing high-quality care and flexible payment options for all our patients. If you have any questions about financing the treatment your doctor recommends, just give us a call—we’re here to help!


Please note: Payment is due at the time of service. As a courtesy to you, we will bill your insurance company and track claims.


However, we do not belong to any participating provider or group networks, and as of January 1, 2026, we will no longer be a participating provider for Delta Dental Insurance, but we will continue to file their claims.


Please bring a copy of your insurance card for your first visit and keep us informed of any changes to your insurance plan.


You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks.


Any remaining cost is your responsibility. Patients with co-pays, deductibles, percentages, etc., are expected to pay at the time of their appointment.


Any extra funds that are paid will be refunded/returned to you, the patient.
